• Single dose: 4.5 hrs
• Repeat dose: 7 hrs

Metabolism

• Renal excretion
• ~20% removed by HD

Dosing – Adult

• S.C.: 1 mg/kg Q 12 hrs or 1.5 mg/kg Q 24 hrs**
• Obesity: Use actual body weight to calculate dose

Monitoring

• Consider Antifactor Xa level 4 hrs after 3rd dose for weight >120 kg or <60 kg

*Lower doses may be considered for patients at risk for bleeding and stroke patients

**Favor twice daily dosing for patients with cancer or high risk of bleeding or thrombosis. For patients who are obese use actual body weight to calculate dose.

***Obese patients may require a decreased starting dose if dosing is based upon their actual body weight. Consider starting these patients at 12 IU/kg/hr based on actual body weight and titrating the dose based upon UFH levels.
